Initial Screening Process

1. The Greenhouse Recovery Affiliate obtains assessment information from the resident and will verify the identity of the resident through identification.

2. The Greenhouse Recovery Affiliate will have the resident complete and sign the Resident Information Form (See application attached)

3. The Greenhouse Recovery Affiliate will review the Confidentiality policy and obtain the resident’s signature on the Confidentiality Agreement and Release of Information forms.

4. The Greenhouse Recovery Affiliate will also review the resident’s rights and responsibilities, the rules, groups, emergencies, and all other general information found within the program description and obtain all signatures requested.

5. The Greenhouse Recovery Affiliate will provide the resident with a copy of the Resident Handbook and at this time will search the resident’s belongings for any hazardous items.

6. The Greenhouse Recovery Affiliate will administer a urine drug screen.

7. The resident will be informed of the fees for the recovery residence. The current program fee: $160 a week for a shared room.  The resident will then be escorted to their residence and assigned a room.
